Food Safety Level 2 online training is being offered by Ohio State University Extension in partnership with TAP Series. This program meets Ohio Department of Health requirements for Level 2 Certification in Food Protection.
Registration fee of $150 is non-refundable for both the online class and exam. There is an additional fee of $25, if the selected exam date is rescheduled. The ServSafe Manager exam is proctored by an OSU Extension educator who is a Certified ServSafe® Proctor. Tests will be administered at the OSU Extension Office, Scioto County, 602 7th Street, Courthouse Rm #7, Portsmouth, OH 45662. Registration forms at least 2 weeks prior to testing date.
1.Each individual registered will receive 2 email messages (one from TAP and one from OSUE) with username/password and additional class information allowing access to the online class.
2.Student takes the online class (about 2 -4 weeks) and prints the Certificate of Completion.
3.Before or on the exam date, student brings (mails, emails, or delivers prior to taking exam) a copy of the Certificate indicating successful completion of the online TAP course./
4.Student takes the ServSafe Manager Exam on pre-selected date at the Scioto County OSU Extension Office, 602 7th Street, Courthouse Rm #7, Portsmouth, OH 45662. This exam will be proctored by OSU Extension staff who will submit the exam answer sheet to the National Restaurant Association to be graded.
5.When the student passes the exam, a ServSafe certificate can be printed at www.ServSafe.com
6.Upon passing the exam, student must complete an “Application for Reciprocity of Ohio Department of Health’s Certification in Food Protection” form found at: http://go.osu.edu/reciprocity_level2foodsafety
•On this form, supply name of course (TAP) & hours spent on course material (minimum is 15hrs).
•Supply copy of certificate from ServSafe showing successful completion of exam.
